Overview
This form may be used by researchers at the University of Dayton who want to conduct anonymous paper surveys, confidential online surveys, or non-sensitive interviews using only adult subjects. Any other type of research must use the appropriate exempt or non-exempt form available in the Related Services section below. This application form may NOT be used if the research is sponsored with federal funds, if prison populations are used, if compensation is involved, or if minors under the age of 18 are involved. If you are a student, you must ask your faculty mentor to approve the research and submit their proof of approval, along with the Invitation to Participate/Information Sheet, and list of survey/interview questions.
ATTENTION: The University of Dayton is transitioning its IRB application system from TeamDynamix to Cayuse Human Ethics. Please note the following important dates:
- Beginning August 18, 2025, all NEW IRB applications must be submitted in Cayuse Human Ethics.
- Beginning September 15, 2025, if any CHANGES/AMENDMENTS to an existing approved study in TeamDynamix are needed, the study must be submitted through Cayuse as a “New Submission”.
- Applications that are currently IN PROCESS and being reviewed in TeamDynamix will continue through approval in TeamDynamix.
