Use this form if you’re undertaking federally funded classroom research, surveys, or non-sensitive interviews with adult subjects. This form can also be used for classroom research, surveys or non-sensitive interviews with adult subjects when compensation or incentives are offered.

ATTENTION: Before starting this application please have all of your research documents ready for upload and already reviewed by your faculty advisor. Once you begin the submission process you will not be able to save it for future viewing or editing. This portal is used just to gather basic information about your study that should require no more than several minutes of your time, then you will attach any relevant proposal documents for submission.
