Institutional Review Board (IRB) Non-Exempt Application

Overview

The IRB reviews all proposed university research projects involving human subjects to ensure that the rights and welfare of research subjects are protected as required by federal guidelines.

Use the non-exempt form if your project does NOT fit the description for Fast-Track application forms or general exempt application forms as described on the IRB Forms and Documents page.

ATTENTION: The University of Dayton is transitioning its IRB application system from TeamDynamix to Cayuse Human Ethics. Please note the following important dates:

  • Beginning August 18, 2025, all NEW IRB applications must be submitted in Cayuse Human Ethics.
  • Beginning September 15, 2025, if any CHANGES/AMENDMENTS to an existing approved study in TeamDynamix are needed, the study must be submitted through Cayuse as a “New Submission”.
  • Applications that are currently IN PROCESS and being reviewed in TeamDynamix will continue through approval in TeamDynamix.
