As a Catholic and Marianist University, the University of Dayton is committed to providing reasonable workplace accommodations to ensure employees with qualifying disabilities enjoy access to employment opportunities.
Use this form to request a workplace accommodation due to a qualifying disability under the Americans with Disabilities Act (ADA). A workplace accommodation is a modification or adjustment to a position, an employment practice, or the work environment that makes it possible for a qualified individual with a disability to perform the essential functions of the position.
Completing this form initiates the interactive process, a collaborative effort between you and the University to identify reasonable workplace accommodations that may enable you to perform the essential functions of your position.
NEXT STEPS: After you complete the Employee Accommodation Request form, your health care provider must complete the Accommodation Request Medical Cert Form (.pdf). This form should be submitted directly to Human Resources by uploading a copy to the Employee Relations Secure Document Upload folder, faxed to (937) 229-2569, or placed in the secure HR drop box outside the HR main office 315 St. Mary’s Hall.
