Use this form to indicate the executive and management changes for Flyer Enterprises to implement the transition to new leadership. This transition typically occurs at the end of March or the beginning of April. This form should not be used for any other purpose. If you have mid year changes, use the other student related TDX forms including Report a Hire, Report a Student Termination and the Student PAF.
