Use this form to verify the date(s) of a student appointment at the Student Health Center.
Students are responsible for notifying their professors about absences due to short-term illness or injury.
The Student Health Center does not provide written excuses for illness or injury.
Faculty may use this form to confirm that a student has been in contact with the Student Health Center. Specific details about the visit will not be provided.
Note, students who visit the Premier Health Student After Hours Clinic on Brown Street may receive written documentation from that office. The Student Health Center does not have access to Premier Health records and cannot confirm visits to that location.
