Use this form to request an update to your graduation application after your initial submission.

If you did not apply by the graduation application deadline per the Academic Calendar, you must speak with your Dean's Office so they can review your degree audit for your graduation application to be moved. 

Please speak with your Dean's Office if you need to make updates to your major(s), minor(s), concentration(s), etc. on your graduation application.  They must review your degree audit for the changes to occur.
