Service Overview
Use this form to request the release of information held by the Office of Learning Resources (OLR) related to disability accommodations and needed for things like transferring to a new school, applying to another program, or for proctored examinations by third-party entities (GRE, MCAT, State Teaching Licensure Exams, etc.).
Standard turnaround time on requests is 10 business days. If records are needed sooner, specify the reason on the form.
You will be contacted to verify your identity by a member of the Office of Learning Resources. Failure to complete the verification step will delay processing your request.
