Graduate Course Retake Approval

Service Overview

In accordance with the Graduate Retake Policy, the Graduate Program Director or Department Chair must submit this request form to initiate the grade exclusion designation and subsequent recalculation of the student’s cumulative quality-point average with the Registrar’s Office within 30 days of the student completing the retaken course.
