Service Overview
Use this form to consent or decline the Hepatitis B vaccination provided by the University of Dayton. This service is for employees enrolled in the Bloodborne Pathogens program whose job duties carry a potential risk of exposure to blood or other potentially infectious materials.
Employees who decline the vaccination must complete a mandatory declination statement as required by OSHA standards and the University’s Exposure Control Plan.
