Hazardous Material Shipping and Transportation Request

Service Overview

Completion of Hazardous Materials Shipping (Awareness) training in SciShield is required prior to submitting this request.

Environmental Health and Safety (EHS) processes and approves all hazardous material shipments or transports. Use this form to request EHS processing and approval for hazardous material shipments and transportation, including non-regulated biological, radiological, and limited quantity packages.  A Safety Data Sheet (SDS) or equivalent documentation must be included to support proper classification and handling.  Once a request is received, EHS will reach out to schedule a time to secure the material being shipped and package the material according to DOT requirements.  Standard turnaround time is 3–5 business days.
