Service Overview
Use this form to submit a request when an external organization requires a UD Certificate of Insurance for an event or activity.
Submit this request to Risk Management and Insurance (RMI) at least three business days prior to the date the certificate is required, along with any written contract or agreement between the University of Dayton and the outside agency.
