Use this form to request the addition of ProctorTrack to your Isidore course site and/or request the configuration of an existing online assessment to ProctorTrack. You should only use ProctorTrack for high-stakes exams in fully online courses.
You must submit your ProctorTrack request at least one week before the online exam to allow time for student onboarding in the ProctorTrack system.
