Faculty Leave and Sabbatical Request

Service Overview

Full-time faculty use this form to request a leave for the following reasons:

  • Birth of a child or to care for a newborn child
  • Applying for a sabbatical (invited/qualified tenured faculty only)
  • Non-FMLA personal/research leave

Attach or provide any information that may be appropriate to substantiate the request for the leave.

  • Birth of a child or to care for a newborn child: You MUST attach your modified duties documentation in this form. The University of Dayton Medical Certification Form (.pdf) MUST be completed by the attending healthcare provider and submitted directly to Human Resources via email to benefits@udayton.edu, faxed to 937-229-2569, sent via campus mail to campus zip +1649, or delivered in person to 315 St. Mary’s Hall.
  • Applying for a sabbatical: You MUST attach the Sabbatical Plan, approval/feedback documentation from the department sabbatical review committee, and any additional approval/feedback from the department chair or dean.
  • Non-FMLA personal/research leave: Attach a statement detailing the reason for your leave request and any other information for verification as requested by the University. For research leave requests, include the institution where study will be undertaken. Such leave requests may be granted on a case-by-case basis without pay and generally require the employee to have completed one year of service. Refer to the Leave section of the appropriate Benefit Handbook for more information.
