Use this form if you are planning a non-credit learning opportunity such as a workshop, training, seminar, institute, or conference and need approval to issue a Certificate of Attendance or Completion.
When to use this service:
- Your program offers non-credit professional development for educators, professionals, or community participants.
- Participants need basic proof of participation or completion (listing hours, dates, and topics).
- You are not requesting official University Continuing Education Units (CEUs) or academic course credit.
Need to award official CEUs for professional licensure? Use the Continuing Education (CEU) Course Proposal service instead.
