Report Receiving the COVID-19 Vaccine


UD students and employees should use this form to report they’ve received their full COVID-19 vaccination and upload a copy of their vaccination record. NOTE: If you are receiving a two part vaccination, please report only after you’ve received the second dose. 

Proper documentation of COVID-19 vaccination is essential for determining potential quarantine needs.

Information submitted by or about you will be treated with respect to your privacy as much as possible.  However, we occasionally may be required or permitted by applicable federal, state, or local laws to reveal information from your health record. Examples include but are not limited to: disclosing to public health authorities and/or school officials in case of certain communicable diseases; in certain emergency situations (if FERPA information) or to emergency contacts.
