This site has been established for members of the UD community to report COVID-19 related health conditions. Use the forms below to:
1. Self-disclose your positive COVID-19 test result (students, employees) (if you are not a student or employee please email covidhealthreporting@udayton.edu with your name and phone number and a contact tracer will call you)
2. Report a close contact exposure to UD if you've been contacted by Public Health (students, employees)
3. Provide daily symptoms updates if you're a UD student in quarantine / isolation
4. Provide a "final day" symptoms update if you're a UD employee on your last day of quarantine / isolation
For medical emergencies, call 937-229-2121 (Public Safety) for on campus and 911 for off campus. If you are a student who wants to talk with someone from the Health Center, call 937-229-3131. UD students and employees should use this form to report they’ve received their full COVID-19 vaccination and upload a copy of their vaccination record.
If you have been diagnosed with COVID-19, use this form to report the diagnosis to the University for isolation and contact tracing purposes.
UD students in quarantine / isolation should use this form to report COVID-19 symptoms daily.
Employees should use this form to report COVID-19 symptoms on their final day of quarantine / isolation.
A member of the UD community was determined to be a close contact with any individual that has tested positive for COVID-19
If you are not a member of the UD community and have been diagnosed with COVID-19, use this form to report the diagnosis to the University.
