Employee Quarantine / Isolation COVID-19 Symptoms Check-In

Service Overview

Prior to returning to campus, employees under quarantine or isolation are required to report if they have had any COVID-19 related symptoms in the past 24 hours. Use this form to complete a symptom check-in on the final day of your quarantine / isolation. 

Information submitted by or about you will be treated with respect to your privacy as much as possible.  However, we occasionally may be required or permitted by applicable federal, state, or local laws to reveal information from your health record. Examples include but are not limited to: disclosing to public health authorities and/or school officials in case of certain communicable diseases; in certain emergency situations (if FERPA information) or to emergency contacts.
